Connecting Care is now CENTRAL CONNECT

The Tasmanian Anticipatory Care Project has concluded. Final Reports are at Anticipatory Care Project | Department of Health

Connecting Care leaves two significant legacies –

  1. The Connecting Care Portal including the expanded Local Resource Directory which has been gifted to Central Coast Council
  2. The Central Coast Community Wellbeing Governance model which now underpins CENTRAL CONNECT.

Connecting Care and Collective ed. have partnered to implement a single, sustainable community governance model for Central Coast to avoid unnecessary duplication.

CENTRAL CONNECT is about local people taking a local approach to improving health, education and connection across Central Coast to address wellbeing concerns and improve connection through a Leadership Roundtable. The Roundtable is responsible for supporting the work of Action Groups, established to achieve outcomes for community.

The governance model provides an avenue for Central Coast Council, health, education, social care agencies, businesses and industry to communicate and connect.

Connecting Care’s focus continues through the CENTRAL CONNECT Chronic Health Action Group.
