Baptcare Horizons

Horizons is a short term, Psychosocial Outreach Program providing 1:1 Support but also running group sessions.
Coordinators work with participants for around 1-1.5hrs per week, the program is goal focused with a large focus on safety planning.
Referrals can be through a GP, Community Service Provider, Psychologist etc, Referrals must come from a service provider.
Participants can be case managed by Adult Community Health Services but cannot have NDIS.
Participants cannot be working with any other Psychosocial Support Program to be eligible for Horizons.
